The Meridian model

The hard case should not feel like a maze.

One fictional nurse navigator connects the plan across specialties, locations, and recovery milestones. The experience is designed around three visible handoffs.

  1. 01
    Listen once

A shared intake frames the need without asking the patient to rebuild the story at every door.

  2. 02
    Confer together

Specialists review the same timeline before proposing the next fictional step.

  3. 03
    Leave with a map

The portal keeps results, visits, preparation, and one care-team message in view.

Common questions

Before you use the demo

Is Texas Meridian a real hospital?

No. Every location, clinician profile, service description, review, metric, appointment, and patient record is fictional demonstration content.

Does the Find Care form book anything?

No. The server validates the fictional fields and returns a simulated reference. It does not persist the submission or contact any provider.

What is stored in the patient portal?

A D1 database contains one seeded fictional patient chart with sample results, appointments, and messages. Sign-in is handled only by Pocket ID through Cloudflare Access, so the site stores no passwords. Every authorized Pocket ID user sees the same fictional chart. Never enter real health data.

Can the Care Navigator interpret my symptoms or results?

No. It is grounded in this website’s fictional content and is instructed not to diagnose, interpret results, recommend treatment, or replace a clinician.
